Sciatica is a common problem for manual workers, sedentary office workers and can occur during pregnancy.
Sciatica nerve pain is caused by pressure on the sciatic nerve – a very large nerve that originates from the lower lumbar spine and travels through the buttock into the leg as far as the back of the knee.
Pressure on the Sciatic nerve can result from a number of reasons including:
* piriformis syndrome (tightness of the piriformis muscle in the buttock that compresses the sciatic nerve)
* spinal misalignments, vertebral dysfunction
* herniated disc, disc prolapse
* poor posture – wearing high heels, prolonged sitting, poor mattress
* poor lifting technique and poor bending habits
* spinal compressions due to osteoporosis
Since there are many disorders that can cause sciatica, or conditions that mimic sciatica, your physiotherapists’ first task is to determine the likely cause of your symptoms. Your physiotherapist will take a detailed history from you and perform a full physical examination, looking at your movement, strength, flexibility and nerve function. Your physiotherapist may refer you for an X-ray or other imaging, but this is infrequently needed.
As sciatica is due to pressure on the sciatic nerve, it stands to reason that treatment involves removing this pressure. Your physiotherapist can help with this with treatments including spinal and nerve mobilisation, massage therapy, exercise rehabilitation and postural training. In addition, your physiotherapist will give you advice as to what you can do to help with your symptoms, such as through the use of heat/cold packs, postural advice and home exercises.
