About Back Pain

Lower back pain is an extremely common condition with up to 80% of the population experiencing back pain at some time during the lives. Although the underlying pathology of low back pain is usually not serious, it is an important cause of pain in the community and has considerable impact on the economy due to time taken off work. Whilst the vast majority of patients with back pain will recover within 6 weeks, a small number will go on to develop chronic or ongoing back pain.

Most causes of back pain are not serious, and relate to the muscles or joints of the lower back and pelvis. The nerves of the lower back can sometimes be affected, whilst sometimes pain can be ‘referred’ from other areas. Lower back pain can be acute, following a one-off incident, or more gradual, following a history of ‘wear and tear’ over time.

Other causes of lower back pain include inflammatory arthritis (e.g. ankylosis spondylitis, Rheumatoid arthritis), and fractures due to osteoporosis. Much less common conditions causing low back pain include cancer or serious infections.

Assessment

Your Physiotherapist will take a detailed history from you about your pain, including when it started and what makes it better or worse, and conduct a full examination of your posture, movement and structures of your low back to determine why you are likely getting your pain, and what can be done to help. X-rays and other imaging tests are infrequently needed.

Treatment

Physiotherapy treatment of low back pain generally focuses on restoring movement and function to the joints, as well as reducing muscle tightness, increasing muscle strength and improving posture. This can result in improved movement, reduced pain and increased function.

Treatments are tailored to the individual, and may include techniques such as spinal mobilisation and soft tissue therapy, muscle energy techniques, dry needling, core stabilization, strengthening or stretching exercises, and postural advice and training.
